Job summaryTo work as part of a team to provide early intervention, prevention, outreach nursing support for people who are experiencing homeless or rough sleeping across Somerset. This includes those in hostels and supportive or temporary accommodation.You will work within the neighbourhood and primary care directorate, supporting people who are experiencing homelessness. Providing a person centred service, you will be working collaboratively within a multi-agency environment along with colleagues in primary and secondary care and voluntary agencies. You will deliver care to the patient where it is needed, exercising compassion and advocacy at all times.Main duties of the jobThe post holder will actively engage and work with a small number of adults with co-existing mental health and substance misuse needs and who is homeless or in temporary accommodation and who at times present with extremely challenging behaviours.
