Job summaryThis post is to support a new initiative working more closely with our community optician partners to support patients who can be safely cared for by community opticians whilst also allowing patients to be seen closer to home.This post offers an exciting opportunity for an experienced administrator to assist in managing the safe discharge of selected ophthalmology patients to the care of our community partners ensuring all required information is transferred to the community practitioners in a timely manner.The role will involve using the community optician's own bespoke IT system. The postholder will also be responsible for maintaining accurate records of discharged patients to facilitate accurate reporting and auditing of this service. The role is essential to ensuring patients are managed appropriately and do not end up lost in the system. The role will then cross reference this with other data held on hospital systems to confirm which patients require treatment and when to uphold treatment plans within the clinically advised timeframe (as advised by clinicians).Many patients will change their contact details. The role will liaise with this complex group of patients to identify up to date information and facilitate their access to treatment.This will require caring, patient and tactful communication to help patients complete their treatment journey. For some patients this is often diagnosed later in life but they will remain a patient under our care long term leading to an ever increasing cohort of patients.
