A fantastic opportunity has arisen for a Band 4 Dietetic Assistant Practitioner to facilitate delivery of evidence based interventions as part of the Level 2 Weight Management Service. This is a Healthboard wide service that aligns to the All Wales Weight Management Pathway so the ability to travel between Healthboard sites and community venues is essential. Main duties of the job You will follow standard operating procedures and contribute to the aims and outcomes of the Dietetic Service by providing support and assistance to Dietitians working within the weight management service. You will plan and prioritise patient caseload and implement weight management support for patients .This will take the form of groups and individual consultations, virtually, in community venues and in people's homes. Supervision from Dietitians is available for reference and guidance. You will enable, support and assist patients who are receiving individual treatment programmes which have been agreed with and planned by Dietitians.
