Job summary An exciting opportunity has arisen for an Advanced Clinical Practitioner at Boroughbury Medical Centre. We have been awarded an Outstanding rating by CQC Main duties of the job Key Responsibilities of the Advanced Clinical Practitioner: Comprehensive Patient Assessment: Conduct thorough assessments, including physical examinations, patient history taking, and interpreting medical tests, to diagnose and manage a variety of conditions. Prescribing and Treatment Planning: Prescribe medications, request appropriate investigations (e.g., blood tests, X-rays), and develop comprehensive treatment plans in partnership with patients. Patient Education and Counselling: Provide patients with information and advice on their conditions, medications, and treatment options, empowering them to actively participate in their care. Collaboration and Communication: Work effectively within a multidisciplinary team, communicating with other healthcare professionals (e.g., GPs, nurses, specialists) to ensure seamless patient care. Managing Complex Cases: Demonstrate expertise in managing patients with long-term conditions, complex health needs, and acute illnesses. Mentorship and Support: Support and mentor other members of the nursing team, including Practice Nurses and Healthcare Assistants, contributing to their professional development.
