The Hub caters for service users with all diagnostic categories and is a needs-led service. Within the Hub we are using Dialog+ to plan care and as part of our Think Family approach are actively engaged with families and carers. In line with the parity of esteem model the Hub supports the physical health management of patients with dedicated nurses for this purpose. Main duties of the job Leadership The post-holder will have Consultant and RC responsibility and will be supported by a first-class multi-disciplinary team.
