Job summaryAre you a Band 5 Nurse who would like to join our dynamic team? We are seeking an ambitious, motivated, compassionate and experienced Band 5 nurse to support the changing face of community health care. As part of the Urgent Community Response and Virtual Ward initiative, your role will be essential in providing efficient, integrated, urgent care to our patients with care and respect.You will be working as part of the Primary Care at Home team predominantly on Unplanned Care to triage and assess referrals into the service; You will be accountable for initial history taking, clinical assessment, triage and onward referral. There is an expectation that with demand in planned care you would be flexible to cross cover.Through face to face patient visits you will undertake a full holistic assessment, plan care with the patient at the centre and undertake physical examination of the patient with undifferentiated conditions.Main duties of the jobYou will be working closely with other members of the MDT and partners to formulate a management plan that will support the patient to remain in the community.Experience in the field of adult community services is desirable but a willingness to learn and contribute is more important. Consideration will also be given to staff who are newly qualified and are passionate about a career in community nursing.
