Job summaryAn exciting opportunity has arisen for a dynamic and experienced Senior Nurse with a District Nursing qualification, to join the community nursing team in Enfield.The District Nursing Team manager is responsible for the health care management of patients in a particular geographical area, together with responsibility for the leadership and management of the team members in the area.The post holder will be responsible for managing the team and working with the Caseload Manager to ensure the staff provides a model of high-quality holistic nursing care to patients in the community.Also be required to contribute to meeting local and national performance targets. It is essential you are able to lead and implement change in order to continuously improve the efficiency and enhance Enfield's patient experience.A key role will be managing patients with complex care needs and working with Hospitals to facilitate smooth transition of care into the community.
