Job summaryThe Pharmacy Service is comprehensive and we have a full range of services including Inpatient and Outpatient dispensary service, ward-based clinical pharmacy, Medicines Information, aseptic and non-sterile production services. Specialist clinical pharmacists provide direct clinical input into medicines management in a number of clinical areas including HIV, general medicine, emergency services, surgery, rheumatology, anti-coagulation, oncology & haematology, women's health, paediatrics and infection control.Main duties of the jobIn conjunction with the Chief Pharmacist will lead on patient safety and clinical governance initiatives relating to medicines management, including EPMA and Controlled Drugs, across the Trust. The post holder will assist the Chief Pharmacist develop, deliver and evaluate the medication safety programme for the Trust.will lead on the review and evaluation of clinical risk and critical incidents, including trends, relating to medicines use within the Trust.will be responsible for dissemination of learning relating to risks, including risk prevention.will contribute to the production of departmental and Trust policies, guidelines, Standard operating procedures and bulletins, relating to patient safety and clinical governance, as appropriate.will be a member of a number of groups and committees concerned with patient safety, medication safety and clinical governance within NMUHT and also across the sector.will coordinate the delivery of a responsive, proactive, patient-focused clinical pharmacy service to ward(s) as required. This will include the promotion of safe, effective and cost effective drug use to facilitate patient admission and enhance continuity of care across the primary/secondary interface, ensuring appropriate mechanisms are in place to promote high quality drug use, effective risk management, patient safety and adherence to trust policies and guidelines.
