Job summary12 month secondment or fixed term opportunity to become an integral part of our dynamic Urology team.As a member of our team, you'll play a crucial role in independently supporting the clinical nurse specialist team while aiding in the planning of assessments for patients on suspected cancer pathways.Your responsibilities will encompass a range of vital tasks, including managing systems for handling results, facilitating communications with patients as needed, scheduling appointments, and monitoring and delivering clinical outcomes. Collaborating closely with the Urology CNS team, you'll contribute to enhancing FDS and 62-day targets, ultimately elevating the standard of patient care.****Please note we are not eligible to sponsor for this post****Main duties of the jobThis role will cover a range of administrative tasks including coordinating necessary assessments, investigations, and appointments for patient pathways.Proactively ensure patients have received eGFR results before MRI and PSA results before review clinic to prevent pathway delays.Facilitate care transfers between service teams and across care interfaces.Refer and signpost patients for higher-level decisions and support.Triage incoming patient calls, initiating appropriate responses based on established protocols.Monitor, audit, and report on service, inputting data to Somerset cancer database.Conduct pre-planned outbound calls to patients under healthcare professional guidance.Contribute to clinical/quality management team reviews of pathway issues.
