Job summaryAn exciting opportunity has arisen for an Occupational Therapist to join our Primary Care Home team delivering high quality Therapy at home to patients registered with GP surgeries in South Waveney. The PCH team works closely with GPs and other members of the multidisciplinary team to provide an excellent standard of person centered care to adult patients in their own homes, residential care or clinic settings, supporting patients to self-manage whenever possible. The service uses SystmOne as a clinical IT system to support mobile working.The successful candidate will be accountable for the effective management of a caseload, ensuring the delivery of high standards of evidence-based therapy input. They will demonstrate excellent leadership, organisational and communication skills and the ability to influence and manage change.Occupational Therapy is challenging, varied and rewarding; the service has to be flexible to meet the needs of patients.
