Job summaryOffered as a permanent contract, the role will work across the week within normal practice opening hours (0800-1830). Start and finish times are flexible. Working condensed hours (eg. full time in 4 days) will be considered where requested (subject to service need).An Advanced Clinical Practitioner is a registered Health Care Professional. You will be a Nurse, Paramedic or Pharmacist qualified with an appropriate Master's Degree in Advanced Clinical Practice relevant to Primary Care.You will be an experienced ACP within primary care who is able to deliver quality individualised care to our patients with a high level of autonomy underpinned by the four pillars of clinical practice, leadership and management, education, and research.You will be expected to utilise your skills in advanced clinical assessment and diagnostic reasoning to make complex decisions to determine and initiate changes to treatment, in collaboration with a multidisciplinary team and supported by a clinical supervisor.You will have the knowledge and experience to be able to supervise other clinical members of staff within the PCN Team.Main duties of the jobThe role will include some/all the following depending on hours worked:Home visits to acutely unwell patientsReview of long-term conditions for house bound patients where clinically appropriateSupervision/point of contact to our PCN Paramedic and Nursing/HCA home visiting serviceSupport/supervision/education to our other patient facing rolesSupporting our care homes alongside local GPs/Pharmacists and Nursing TeamSome in Practice clinical sessions dependent on service needAbout usOur PCN consists of 5 GP Practices: Alwoodley Medical Centre, Meanwood Health Centre, North Leeds Medical Practice, Street Lane Medical Practice and Diamond Medical Group, based over 7 sites, caring for a population of over 80k patients.We work together with a range of local providers, including community services, social care and the voluntary sector, to offer more personalised, co-ordinated health and social care to our local population.We currently employ a team of 45 including:Pharmacists and Pharmacy TechniciansHealth and Wellbeing CoachesParamedicsNurse Associate & Trainee NA plus Health Care Assistants/phlebotomistOccupational TherapistsSocial and Wellbeing PrescribersGPs (in Advisory and Supervisory roles)Admin & Management SupportJob descriptionJob responsibilitiesMAIN TASKS:CLINICALWork within the multi-disciplinary team to provide urgent care and a home visiting service, providing clinical triage when required.Provide safe, evidence-based, cost-effective, individualised patient care within the surgery, patients own home or other environment where patient care is carried outClinically examine and assess patient needs from a physiological and psychological perspective, and plan clinical care accordingly referring patients to other services/agencies in a timely manner, utilising Practice, PCN and local guidelines.To recognise the need for and be able to perform and interpret investigatory procedures, including laboratory and radiology.Prioritise health problems and intervene appropriately to assist the patient in complex, urgent or emergency situations, including initiation of effective emergency care.Support the delivery of anticipatory care plansSupport patients to adopt health promotion strategies that promote healthy lifestyles and apply principles of self-care.Manage mental health needs and referring on as clinically appropriateRecognise and work within your own competence and in accordance with professional codes of conduct, maintaining accurate and contemporaneous health recordsIndependently prescribe for patients within your scope of practice. Using your knowledge, you will provide information and advice on prescribed or over-the-counter medication.
