Job summaryA newly created and very exciting opportunity has arisen for the role of a Band 6 Paediatric Special School Liaison Nurse.Are you highly motivated, passionate and can deliver appropriate professional health advice by supporting our Special Schools within the local authority and the wider multi-disciplinary team?The successful post holder will work automatously and take a proactive approach as the key liaison nurse and facilitator for the child/young person between Education and Health care settings.You will have a good understanding and knowledge of the Special School Nursing Framework for Wales and the ALNET (Wales) Act 2018.This will ensure only the highest standards of care is delivered, monitored and evaluated. You will be extremely well supported by our excellent existing Integrated Children's Community Nursing Service (ICCNS) although you will work mainly between the Special School Nursing and Education teams.Main duties of the jobSummary of Role PurposeProvide and evaluate a new nursing model for the Schools which provides a nurse liaison role.Support the schools to quality assure their policies & procedures which relate to children's health and well-being needs.Support the schools with record keeping for the management of learner's healthcare needsProvide the schools with support and advice regarding medication management (storage, access, administration, access to training and recording).Support the schools to access relevant training to maintain staff's competence in dealing with health-related emergency procedures.Liaise with the learner, families and other healthcare professionals to co-ordinate and share information to support the school in meeting individual healthcare plans.Work with the schools, child and family, and wider multi-agency team to support young people transitioning to adult services.Empower learners to support their own health care needs, wherever possible, exploring adapted training and support to meet individual need.The post holder will have a sound knowledge of Children's Rights, Safeguarding & ALN knowledge in order to support schools in promoting this and raising awareness with other professionals.The ability to speak Welsh is desirable for this post; Welsh and/or English speakers are equally welcome to apply.About usCardiff and Vale University Health Board is one of the largest Integrated Health Boards in the UK, employing over 15,000 staff, providing over 100 specialist services.
