We are looking for a dynamic, innovative and motivated person to join The Community Rehabilitation Enhanced Support Team (CREST). CREST is a new service within the mental health complex rehabilitation pathway specifically developed to provide different ways of working with individuals who have spent long periods of time in hospital.The aim of the service is to provide psychologically informed, flexible and intensive packages of support aimed at building service user confidence to live a meaningful life in the community as an alternative to a hospital complex rehabilitation placement. The service also facilitates system change by providing a consistent approach to community rehabilitation and safety planning that supports more hospital providers to make discharge decisions. The team work collaboratively with users and partners from local multi agency providers and services.Main duties of the jobCREST is a multi-disciplinary team including colleagues from third sector organisation. We offer a seven day a week service over extended hours, currently covering Calderdale, Kirklees and Leeds but working towards extending service delivery to Wakefield and Bradford by the end of year 2.The successful candidate will be providing vital administrative support for CREST (community rehabilitation and enhanced support team). CREST is part of the mental health complex rehabilitation pathway established in 2021 working closely with service users and families to empower them define, rebuild, and live, independent and satisfying lives closer to their families and community. You will be directed and guided by the Team coordinator but will at times work using own initiative within the boundaries of the role.We provide a high-quality support service which is integral to the success of the service, and we are looking for someone who feels they can enhance the admin provision already being provided.You will be an experienced team player who has the skill, knowledge, and the right attitude able to assist in the delivery of a high quality, accurate and timely support service.
