About the Team
The successful applicant will become part of an experienced and motivated Mental Health Social Care Management Team. As a Team Manager you will support our social care Team Managers in the North of the County who oversee the work of the dementia, older adult and CMHT social care pathways, as well as our specialist Mental Health carers service. North Warwickshire is a local government district with borough status in Warwickshire, England. The borough includes the two towns of Atherstone and Coleshill, and the large villages of Polesworth, Kingsbury, Hartshill and Water Orton along with smaller villages and surrounding rural areas.
About the Role

This Delivery Lead role is a unique opportunity to build upon our successful relationship with Coventry and Warwickshire Partnership Trust (CWPT) who, like us, are in the final stages of an exciting period of Transformation.
Redefining and consolidating our key social work offer within the wider Community Mental Health Teams has been an integral part of our Transformation vision. Although still seconded to CWPT under a successful section 75 agreement, we now directly line manage our own social resource and focus on meeting the Care Act needs of those experiencing mental ill-health.
As a Team Manager you will work closely with CWPT partners to identify and address social care needs both for new referrals, as part of the triage process, and for those already open to services. You will also have a key link role in facilitating the discharge of psychiatric inpatients by ensuring social care needs are identified as early as possible with a view to accessing support from either our urgent or community social care pathway.
You will be able to demonstrate excellent organisational skills, a head for complexity, and a desire to improve the outcomes and wellbeing of those you work with. Above all we are looking for inspirational leadership from someone truly motivated to improve the outcomes of the people we work with.
If the successful applicant is a qualified AMHP you will be part of a countywide AMHP rota assessing people of all ages who are experiencing a mental health crisis.
