The postholder will provide heart failure and cardiac rehabilitation expertise to support our innovative forward thinking acute and community teams within North Warwickshire. They will demonstrate strong leadership skills and will have the ability to drive services forward in its development including supporting our Acute Coronary Syndrome Cardiac Rehabilitation Teams. Main duties of the job The post holder will exercise advanced clinical skills, levels of judgment, discretion and decision making within both the Heart Failure and Cardiac Rehabilitation Teams and will act as a clinical expert in their field. They will work collaboratively within both the Heart Failure and Cardiac Rehabilitation Teams, GE Home Virtual Wards, within Cardiology, with our Allied Health Professionals, Place Partners, Community Teams and Primary Care with a strong focus on continuing the services focus on supporting health inequalities within the local area. Duties will include acting as a clinical expert and will work across acute and community services i.e. home visiting, supporting community/hospital clinics and the Cardiac Rehabilitation exercise classes. The post holder will have strong leadership and clinical skills to help support the Nursing Teams and Allied Health professionals to develop their skills within cardiology and will provide regular teaching sessions within the Trust to raise awareness of heart failure and cardiac rehabilitation.
