Main duties of the job Provide senior clinical and operational oversight to ensure safe, effective, and continuous Hospital at Home service delivery. Lead the coordination, triage, and timely admission of referrals, ensuring patients access the most appropriate care pathway. Undertake a weekly in-reach rotation at Princess Alexandra Hospital to support early discharge planning, multidisciplinary working, and identification of patients for community pathways under the "Home First" ethos. Work with acute, community, and system partners to prevent unnecessary admissions and support coordinated, safe discharges. Oversee day-to-day operational issues, escalate as needed, and manage caseload flow across the service. Monitor and maintain compliance with staff supervision, appraisals, and mandatory training. Lead and participate in clinical audits, reflective practice, and learning from incidents to maintain high standards of care, governance, and continuous improvement.
