    • The Urgent Care Service are a team of Doctors Nurses AHP’s and health care assistants that provides short term urgent health care support to people in order for them to remain in their own homes or to facilitate early hospital discharges. The Urgent Care Service (UCS) are looking for 3 enthusiastic and adaptable Caseload Assistants to join our caring and progressive teams in Deal, Canterbury and Ashford. We are looking for applicants who have good organisational and coordination skills and share the CARE values of the trust. You must be able to work cohesively within a diverse close-knit team and be able to implement systems and processes that ensure efficient and robust care is delivered to patients in their home environment. Due to the challenging nature of the work we require individuals who can operate in a confidential and professional manner, possess excellent communication skills, including the ability to listen and adapt their working practices when needed in a balanced way. Successful applicants will have the willingness to undertake a varied shift pattern over a 7 day working week, including some bank holidays and weekends and as occasional working across sites is needed, applicants are required to have a full UK driving licence and a vehicle to use. Ensuring that patient visits are booked in and allocated in an efficient way. · Ensuring staff handovers occur in a robust fashion · Use a variety of Electronic systems including Microsoft excel, Word, Teams and our electronic patient record Rio. · Regularly Work within our single point of access team (SPOA) in person and/or remotely. · Liaise with the Caseload coordinators in managing the incoming referrals to the team, · Offering cover to the other locality teams/coordinators when needed. · Perform basic triages of patients using clinical led algorithms. · Work across sites within East Kent when necessary.
