Job summary We are delighted to be able to advertise this position to become part of our integrated, multidisciplinary team covering North East Hampshire. You will be working in the community, treating patients in their own homes across Fleet, Farnborough, Aldershot and Yateley. You will be part of the nursing team, which forms part of our Enhanced Recovery and Support at Home team, along with OTs, Physios, APs, Rehab Support Workers and admin. We are a rehab service, supporting patients at home either to prevent admission to hospital or on their discharge home. Referrals are usually from the wards or from the GP, Paramedic Practitioners, members of the ICT etc. In addition to holistic assessments, primary nursing needs include catheter care, wound care and medication management. Main duties of the job The Nursing Associate will: Be responsible for delivering high quality, patient centred, compassionate care under the direction of a Registered Nurse (or other registered care professional, dependent on setting) with a focus on promoting health and independence Have proficient attitudes and behaviours compatible with NHS Values Be an accountable professional Work as part of a designated clinical and care team delivering care that focuses on the direct needs of the individual Carry out specific clinical and care tasks and responsibilities to a high standard and competency, under the direction of a registered nurse or other registered care professional dependent on setting Be able to work without direct supervision, at times delivering care independently in line with the individual's defined plan of care, within the parameters of practice of the Nursing Associate role, accessing clinical and care advice when needed.
