We are a trans-disciplinary, supportive & dynamic team who work closely with the wider MDT in Emergency & Acute medicine to provide comprehensive and effective patient care & intervention. This includes multi-disciplinary working from the point of admission at triage and ambulance handover with nursing staff and paramedics through to comprehensive geriatric assessment and complex discharge planning with Speciality Consultants and community partners.
